Tooth whitening is one of the most requested procedures in cosmetic dentistry - and one of the most frequently started at the wrong time. Bleaching works best on structurally sound, stable teeth in a healthy oral environment. When those conditions are not present, the results are short-lived and sometimes uncomfortable enough to put patients off treatment for months.
What We Assess Before Recommending Bleaching
- Enamel condition - thin or eroded enamel transmits bleaching agents to dentine faster, increasing sensitivity risk significantly
- Active decay or open margins - bleach entering a compromised tooth causes acute pain and can damage the pulp irreversibly
- Gum health - inflamed gingival tissue does not tolerate peroxide well; gum treatment must precede any whitening
- Existing restorations - composite fillings and ceramic veneers do not bleach; they will appear darker relative to natural teeth after treatment
- Intrinsic staining - fluorosis, tetracycline, and developmental discolouration respond differently and less predictably to external bleaching
In-Office vs. Take-Home: What the Evidence Says
In-office whitening uses higher concentration peroxide activated over 60 to 90 minutes in a single appointment. The result is immediate and often one to two shades beyond what a take-home kit achieves in two weeks. Take-home kits use lower concentration peroxide worn in custom trays for 30 to 60 minutes daily over two to three weeks - gentler, lower sensitivity risk, and more suitable for patients with already-reactive teeth. The best outcomes typically combine both: in-office for the initial lift, take-home for maintenance and future top-ups.
How to Maintain the Result
Whitening is not permanent. Teeth continue to absorb chromogenic compounds from food, drink, and lifestyle. A realistic maintenance plan - periodic top-ups with a clinician-supplied kit, avoiding heavy staining exposure for 48 hours after treatment, and keeping regular hygiene appointments - extends the result significantly.
